(collectively, “Plaintiffs’”) submit this Notice of Errata to notify the Court and
Defendant Life Insurance Company of the Southwest (“Defendant” or “LS W’’) that
the document filed on May 31, 2012 as Document Number 242-1, Part I of the
Exhibits to the Declaration of Dr. Patrick Lee Brockett, is a duplicate of Part I of
the Exhibits to the Declaration of Brian P. Brosnahan (Document Number 241-1)
and was filed in error.

Concurrent with this Notice of Errata, Plaintiffs will file an amended version
of current Docket Entry No. 142, attaching the corrected version of current
